The Port Ministries is located at 5013 S. Hermitage Ave, Chicago, IL 60609, in the Back of the Yards neighborhood.
Street parking is available in the surrounding neighborhood. Please contact our office if you need assistance planning your visit or finding the appropriate entrance.

Free Health Clinic
Tuesdays, 6:00 PM – 8:00 PM
Our Free Health Clinic provides healthcare services to community members. Appointments are preferred, and walk-ins are welcome. No insurance is required.
Patients are encouraged to bring a current medication list and any available medical history or relevant medical records.

Bread Truck
Our Bread Truck distributes free meals and food to individuals and families throughout the community six days a week.
Distribution locations and times may vary. Call 773-280-6267 for current information.
After-School Program
Monday–Thursday, after school
Our After-School Program offers free homework assistance, tutoring, mentoring, and enrichment activities for elementary school students.
Families interested in enrollment can contact our office at 773-280-6267 to learn about registration and availability.
